Things to bring to your first visit
- Your referral slip and x-rays from your referring dentist. Many times your dentist will send us both of these electronically and you won't need to bring them. But if you have them, bring them!
- When you make your appointment, our office will send you a link to the forms you will need to fill out. Please fill them out ahead of time to help expedite your appointment. If you cannot fill them out online, we can assist you when you arrive for your appointment.
- A list of medications you are currently taking (You can also include the list if you fill out the paperwork online).
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
We will complete an in-depth medical and dental health history and a thorough examination in your mouth and work closely with you and your dentist to develop a plan to meet your wants and needs.
